Vista Prairie at Windmill Ponds in Minnesota is hiring an Aspire Care Coordinator to join our compassionate team. You will lead care staff, assist with schedules, and monitor service delivery under the Community Care Manager's guidance.

Deliver care through the Aspire Lifestyle Model to honor residents and enhance their quality of life, and engage them in living with purpose across the Aspire 5 pillars of wellness.

Qualifications

• High school diploma or GED preferred.
• Training in health-related disciplines or relevant experience required.
• RA and/or CNA experience required.
• Leadership opportunity for CNA leadership growth.
• Supervisory and scheduling experience preferred.
• Excellent written and verbal communication.
• Technology proficient.

Responsibilities

• Deliver care through Aspire Lifestyle Model to honor residents and enhance their quality of life.
• Engage residents in living their life with purpose using the Aspire 5 pillars of wellness.
• Demonstrate stewardship and leadership by setting a positive example and boosting team morale.
• Manage care staff schedules and monitor service delivery to residents.
• Follow CCM guidance to meet residents' needs.
