Vista Prairie Communities in Minnesota is seeking an Aspire Care Coordinator to join our compassionate team. You will lead, develop, and train care staff, assist with schedules, and monitor service delivery under the guidance of the Community Care Manager.

Delivering care through the Aspire Lifestyle Model, you will engage residents across wellness pillars to enhance quality of life and provide strong leadership to the team. Full-time day shift with comprehensive benefits awaits.

Qualifications

• High school diploma or GED preferred; relevant health-related training or experience required.
• RA and/or CNA experience required.
• CNA leadership opportunity to grow clinical leadership skills.
• Supervisory and scheduling experience preferred.
• Excellent written and verbal communication; technology proficient.

Responsibilities

• Lead, develop, and train care staff and assist with staff schedules.
• Monitor service delivery to residents under CCM guidance.
• Deliver care through the Aspire Lifestyle Model to enhance residents' lives.
• Engage residents across Aspire 5 wellness pillars (social, physical, intellectual, emotional, spiritual).
