Job summary

Vista Prairie at Windmill Ponds is seeking an Aspire Care Coordinator to join our compassionate team. As an Aspire Care Coordinator you lead, develop, and train care staff, assist with schedules, and monitor service delivery under the CCM, delivering care through the Aspire Lifestyle Model to enhance residents' quality of life.

This full-time day-shift role offers great benefits, including PTO, holiday pay, health/dental/vision insurance, a 401K, and educational assistance.

Qualifications

• High school diploma or GED preferred.
• Resident Assistant (RA) and/or Certified Nursing Assistant (CNA) experience required.
• Supervisory and scheduling experience preferred; strong communication and tech skills.

How you will make an impact:

Vista Prairie at Windmill Ponds is seeking an Aspire Care Coordinator to join our compassionate team. As a dedicated Aspire Care Coordinator you will w ork within the framework of Vista Prairie Communities' values of Caring, Respect, Collaboration, Innovation, and Stewardship. You will demonstrate commitment to build on and help find purpose for each resident, helping them connect to their purpose and enhance their quality of life. This position involves leading, developing, and training care staff, as well as assisting with care staff schedules and monitoring service delivery to residents under the guidance of the Community Care Manager.
Your key accountabilities will include:Delivering and managing care through Aspire Lifestyle Model to honor residents and enhance their quality of life. You will engenage residents in living their life with purpose using the Aspire 5 pillars of wellness, including social, physical, intellectual, emotional, and spiritual.You will d emonstrate stewardship and leadership by setting a positive example, fostering a supportive work environment, and boosting team morale. You must also be dedicated to exceeding expectations to ensure seamless care for residents.As an Aspire Care Coordinator, you will also maintain a high standard of care and professionalism while effectively following guidance of CCM and utilizing resources to meet the needs of residentsand much more! Together, we can make a meaningful impact on the lives of our residents.

Schedule:

Full-Time, Day Shift, 80 hours per 2-week period, EOW, On-Call every 4 weeks for staffing.

What you will need:

• High school diploma or GED preferred
• Training in health-related disciplines and/or relevant experience required
• Resident Assistant (RA) and/or Certified Nursing (CNA) experience required
• Exciting CNA Leadership opportunity to grow and expand your clinical leadership skills
• Supervisory and scheduling experience preferred
• Excellent communication skills, both written and verbal
• Technology proficient
