Job summary

CAN Community Health Inc. is hiring a Patient Access Specialist I to support access to prescribed medications and comprehensive care for patients living with HIV and related conditions.

The role coordinates between prescribers, patients, pharmacy services (including 340B), and financial assistance to ensure timely access to treatment. The ideal candidate will be detail-oriented, compassionate, and experienced in navigating HIV care systems with a focus on equity and patient-centered advocacy.

Qualifications

• Bachelor’s degree or higher in a related field is required.
• 3+ years of relevant work experience preferred.
• Bilingual in Spanish or Creole is required.

Responsibilities

• Coordinate between prescribers, patients, pharmacy services, and financial assistance programs.
• Promote medication adherence by patient education and resolving cost-related barriers.
• Assist with prior authorizations and renewals; support treatment access.
