Qualifications

• Graduate ARNP with WA licensure and prescriptive authority.

• Demonstrated ability to develop teaching plans for patients.

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, and nursing staff on patient care.
• Develop individualized teaching plans to support patient health.
• Maintain chart documentation within 48-hour timeline (95% of the time).
• Meet productivity expectations (3,400 encounters/year full-time).
• Provide care coordination through daily huddles in PCMH model.
• Identify at-risk patients and monitor population health.
