Job summary

Sora Resource Group is seeking an experienced Advanced Registered Nurse Practitioner to join a community-based healthcare organization in Washington. This full-time role operates within a patient-centered medical home model and collaborates with physicians, PAs, ARNPs, and nursing staff to deliver comprehensive care.

Qualifications

• Must be a graduate of an accredited ARNP program.
• Current RN and ARNP licensure in the State of Washington.
• Prescriptive authority granted in Washington State.
• Experience in broad primary care clinic setting is required.
• Experience in minor surgical techniques is preferred.
• Strong knowledge of the nursing process including assessment, planning, implementation, and evaluation.

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care.
• Develop individualized teaching plans to support patient health goals.
• Use independent clinical judgment based on evidence-based data.
• Maintain timely chart documentation within 48 hours, 95% of the time.
• Complete fee slips with correct diagnosis and procedure codes.
• Meet productivity expectations of at least 3,400 encounters per year.
• Provide care coordination through daily huddles within the PCMH model.
• Identify at-risk patient populations and monitor panel health.
• Empower patients through self-management education and discharge planning.
• Advocate for patients by explaining procedures and plans before execution.
• Participate in quality management and peer review.
• Support clinical education of other health professionals including students.
• Maintain required annual training (HIPAA, Cultural Diversity, BBP, Sexual Harassment).
