Job summary

Sora Resource Group is seeking an experienced Advanced Registered Nurse Practitioner to join a community-based health organization operating in a Patient Centered Medical Home model. The role collaborates with physicians, PAs, ARNPs, and nursing staff to provide high-quality care across a diverse patient population.

The ARNP will perform advanced clinical assessments, manage illnesses, and develop individualized teaching plans.

Qualifications

• Graduate of an accredited ARNP program.
• Current RN and ARNP licensure in the State of Washington.
• Prescriptive authority from Washington State.
• Experience practicing in a broad primary care clinic setting.
• Experience in minor surgical techniques is preferred.
• Strong knowledge of the nursing process including assessment, planning, implementation, and evaluation.

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope of practice.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management.
• Develop and implement individualized teaching plans to support patients in achieving and maintaining optimal health.
• Use sound, independent clinical judgment in patient care decisions based on evidence-based data.
• Maintain timely and accurate chart documentation (within 48-hour timeline, 95% of the time).
• Meet productivity expectations (minimum 3,400 encounters per year for full-time).
• Provide care coordination through daily huddles as part of the PCMH model.
• Identify at-risk patient populations and monitor panel-specific population health.
• Empower patients through self-management education and discharge planning.
• Advocate for patients by explaining procedures and treatment plans prior to execution.
