Ashton North LLC is seeking an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based healthcare organization in Washington. This full-time, permanent role operates within a Patient Centered Medical Home model and collaborates with physicians, PAs, ARNPs, and nursing staff to deliver high-quality, comprehensive care.

The ARNP will perform advanced clinical assessments, coordinate care, educate patients, and ensure accurate documentation.

Qualifikationen

• Graduate of an accredited ARNP program.
• Current RN and ARNP licensure in Washington State.
• Prescriptive authority from Washington State required.
• Experience in broad primary care clinic setting preferred.

Aufgaben

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management.
• Develop and implement individualized teaching plans for patients' health goals.
• Maintain timely chart documentation (within a 48-hour timeline, 95% of the time).
• Meet productivity expectations (minimum 3,400 encounters per year).
• Provide care coordination through daily huddles within the PCMH model.
• Identify at-risk populations and monitor panel health.
• Educate patients for self-management and discharge planning.
• Explain procedures and treatment plans prior to execution.
