Job summary

True North Consultants, Inc. is seeking an experienced Advanced Registered Nurse Practitioner (ARNP) to join a patient-centered, community-based health team in Washington.

The ARNP will provide comprehensive primary care within the Patient Centered Medical Home model, collaborating with physicians and staff to deliver high-quality care, coordinate services, and educate patients for better outcomes.

Qualifications

• Must be a graduate of an accredited ARNP program.
• Current WA RN and ARNP licensure with prescriptive authority.
• Experience in broad primary care settings; minor surgical techniques preferred.
• Strong nursing process knowledge: assessment, planning, implementation, evaluation.

Responsibilities

• Perform advanced clinical assessments within ARNP scope.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care.
• Develop and implement individualized teaching plans for patients.
• Use independent clinical judgment based on evidence-based data.
• Maintain timely chart documentation within 48 hours (95% of the time).
• Complete fee slips with correct diagnosis and procedure codes.
• Meet productivity: 3,400 encounters per year (full-time).
• Provide care coordination through daily huddles in the PCMH model.
• Identify at-risk patient populations and monitor panel health.
• Empower patients with self-management education and discharge planning.
• Advocate for patients by explaining procedures and plans.
• Participate in quality management and peer review.
• Support clinical education of students.
• Maintain annual training (HIPAA, Cultural Diversity, BBP, Sexual Harassment).
