Your Talent Team is seeking an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based health organization in Washington. This permanent, full-time ARNP role operates within a Patient Centered Medical Home model and collaborates with physicians, PAs, and nursing staff to deliver high-quality care across a diverse patient population.

Qualifications

• Graduate of an accredited ARNP program.
• Current RN and ARNP licensure in Washington.
• Prescriptive authority in Washington.
• Experience in a broad primary care clinic setting.

Responsibilities

• Perform advanced clinical assessments within ARNP scope.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care.
• Develop and implement teaching plans to support patient health.
• Maintain chart documentation within a 48-hour timeline.
• Meet productivity expectations including annual encounters target.
• Provide care coordination through daily huddles in the PCMH model.
• Identify at-risk populations and monitor panel health.
• Educate patients for self-management and discharge planning.
