Job summary

Talent Solutions Partners seeks an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based health team in Washington. This role operates within the Patient Centered Medical Home model, collaborating with physicians, PAs, ARNPs, and nursing staff to deliver high-quality care across a diverse patient population.

Qualifications

• Graduation from an accredited ARNP program is required.
• Current RN and ARNP licensure in the State of Washington is required.
• Prescriptive authority in Washington State is required.
• Experience in a broad primary care clinic setting is required.
• Experience with minor surgical techniques is preferred.
• Strong knowledge of the nursing process: assessment, planning, implementation, evaluation.

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management.
• Develop and implement individualized teaching plans for patient health goals.
• Use independent clinical judgment based on evidence-based data.
• Maintain chart documentation within a 48-hour timeline (95% of the time).
• Complete fee slips with accurate diagnosis and procedure codes.
• Meet productivity targets (minimum 3,400 encounters per year).
• Provide care coordination through daily huddles in the PCMH model.
• Identify at-risk populations and monitor panel health.
• Educate patients for self-management and discharge planning.
• Explain procedures and treatment plans to patients prior to execution.
• Participate in quality management and department development.
• Support clinical education of students and staff.
• Maintain required annual training (HIPAA, Cultural Diversity, BBP, Sexual Harassment).
