Clear Concepts, Inc. is seeking an Advanced Registered Nurse Practitioner to join a patient-centered, community-based healthcare team in Washington. You will work within the Patient Centered Medical Home model, collaborating with physicians, ARNPs, PAs, and nurses to deliver comprehensive primary care.

Qualifications

• Graduate of an accredited ARNP program (required).
• Current RN and ARNP licensure in the State of Washington (required).
• Prescriptive authority from Washington State (required).
• Experience practicing in a broad primary care clinic setting (required).
• Experience in minor surgical techniques (preferred).
• Strong knowledge of the nursing process including assessment, planning, implementation, and evaluation.

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management.
• Develop and implement individualized teaching plans to support patients.
• Use independent clinical judgment based on evidence-based data.
• Maintain chart documentation within a 48-hour timeline, 95% of the time.
• Complete fee slips with correct diagnosis and procedure codes.
• Meet productivity expectations (≥3,400 encounters per year).
• Provide care coordination through daily huddles within the PCMH model.
• Identify at-risk populations and monitor panel health.
