Acloché LLC is seeking an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based healthcare team operating within the Patient Centered Medical Home model. You will collaborate with physicians, ARNPs, PAs, and nursing staff to deliver high-quality care across diverse populations, while meeting productivity and documentation standards.

Qualifikationen

Graduate of an accredited ARNP program and WA RN/ARNP licensure.
Prescriptive authority licensed in Washington state.
Experience in a broad primary care clinic setting.
Strong nursing process knowledge and clinical judgment.

Aufgaben

Perform advanced clinical assessments and manage illnesses within ARNP scope.
Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care.
Develop teaching plans to support patient health goals.
Maintain timely chart documentation (within 48 hours, 95% timeliness).
Meet productivity: 3,400 encounters per year as a full-time employee.
Provide care coordination through daily huddles in the PCMH model.
Support clinical education of students and staff.
