Job summary

Acloché LLC in Washington State is seeking an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based clinic operating within the Patient-Centered Medical Home model. You will work closely with physicians, ARNPs, PAs, and nursing staff to deliver comprehensive primary care across a diverse patient population.

Qualifications

• Graduate of an accredited ARNP program (required).
• Current RN and ARNP licensure in the State of Washington (required).
• Prescriptive authority in Washington State (required).
• Experience in a broad primary care clinic setting (required).
• Experience in minor surgical techniques (preferred).

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management.
• Develop and implement individualized teaching plans for patients' health.
• Maintain timely chart documentation (within 48 hours, 95% of the time).
• Meet productivity expectations (minimum 3,400 encounters per year).
• Provide care coordination through daily huddles within the PCMH model.
