Job summary

Apple Rehab West Haven is seeking per diem CNAs to assist with resident appointments and floor shifts in a family‑oriented setting at 308 Savin Avenue, West Haven, CT. Starting pay is competitive at $21.22/hour with per diem shifts and weekend coverage requirements.

The role requires CNA certification, attention to detail, teamwork, and the ability to manage routine personal care tasks while reporting changes to supervisors and documenting care and observations accurately.

Qualifications

• Must hold current Certified Nursing Assistant (CNA) certification in good standing.
• No history of patient/resident abuse as per State Registry.
• Proficient in reading, writing, and following oral and written instructions.
• Strong attention to detail, excellent follow-through, and ability to prioritize multiple tasks.
• Team player with good cooperation and interpersonal skills.

Responsibilities

• Assists residents in daily bathing, grooming, dental care, bowel and bladder functions, dressing, walking and wheelchair use.
• Positioning in and out of bed and making beds; records care given per facility policy.
• Observes and reports changes in residents’ condition, including skin breakdowns; reports accidents or injuries.
• Records all necessary charting entries and responds to call lights and emergencies calmly.

Job Opportunity: Appointment Companion - CNA Certified Nursing Assistant

Description: Looking for CNA's who are interested in picking up resident appointment shifts per diem. CNA's are welcome to pickup floor shifts as well.

Starting Pay: Competitive rates starting at $21.22/hour

Available Shifts:

• Per Diem Appointment Companion

Responsibilities: Assists residents in daily bathing, grooming, dental care, bowel and bladder functions, dressing, walking and wheelchair use, positioning in and out of bed. C.N.A. Certified Nurse’s Assistant documents personal care given according to facility policies. Provides nursing functions as directed including perineal care, making beds, turning residents, measures and records temperature, weight, pulse and respiration. C.N.A. Certified Nurse’s Assistant observes and reports to supervisor changes in residents’ condition, including any skin breakdowns. Reports immediately all accidents or injuries. Records all necessary charting entries. Answers all call lights regardless of assignment. Responds to emergency situations calmly and efficiently.

Qualifications:

Additional Expectations: Per diem staff are expected to fulfill (2) weekend shifts monthly to meet requirements.
