Responsibilities

• Conduct video-only virtual visits across multiple practices and regions.
• Provide Urgent & Acute Care encounters via virtual platform.
• Conduct Behavioral Health visits and follow-ups.
• Manage Chronic Disease Management cases.
• Perform Annual Wellness Visits (AWVs) and Annual Exams.
• Complete Transitional Care Management (TCM) visits.
• Maintain EHR Documentation in Epic.
• Minimum of two shifts per calendar month, including one evening/weekend shift.

Job Details:

• Per-diem, 100% remote (work from home) virtual care position
• Conduct video-only virtual visits across multiple practices and regions
• Provide Urgent & Acute Care encounters via virtual platform
• Conduct Behavioral Health visits and follow-ups
• Manage Chronic Disease Management cases
• Perform Annual Wellness Visits (AWVs) and Annual Exams
• Complete Transitional Care Management (TCM) visits

• Minimum of two (2) shifts required per calendar month, consisting of one (1) evening shift or one (1) weekend shift
• Additional shifts may be scheduled based on departmental needs and provider availability
