Qualifications

• High school diploma or GED required.
• Bachelor’s degree strongly preferred.
• Minimum of two years in community outreach or related field.
• Experience with homelessness, serious mental illness, or co‑occurring disorders preferred.
• Bilingual English/Spanish strongly preferred.
• CPRS or Narcan training a plus or willingness to obtain within 30 days.

Responsibilities

• Conduct outreach and home visits to engage AOT participants in housing and employment programs.
• Coordinate with housing, employment, and clinical teams to support treatment and recovery.
• Document outreach activities in the agency EHR and track engagement metrics.
• Collaborate with shelters, hospitals, and community partners to connect participants with resources.
• Provide trauma-informed, culturally responsive outreach and safety planning.
