Job summary

Lead4Life, Inc. in Glen Burnie, MD seeks an Assisted Outpatient Treatment (AOT) Care Coordination Certified Reentry Specialist to support adults with serious mental illness transitioning from incarceration.

You will connect participants to housing, IDs, benefits, healthcare, employment and transportation, coordinating with care teams to promote stability and recovery. This full-time role requires CRS credential or active pursuit, with 2+ years in justice reintegration or related human services.

Qualifications

• Minimum education: High school diploma or GED required; associate or bachelor’s degree preferred in Criminal Justice, Social Work, Human Services, Psychology, or related field.
• Certification: Must hold or actively pursue a nationally or state-recognized CRS credential within 6 months of hire.
• Experience: Minimum 2 years in criminal justice reentry, case management for justice-involved populations, or related human services role.
• Preferred: Bilingual English/Spanish and knowledge of Maryland correctional system is a plus.

Responsibilities

• Provide pre- and post-release reentry planning for AOT participants with correctional involvement.
• Assist participants in obtaining IDs and benefits documents for community reintegration.
• Restore suspended or terminated public benefits (e.g., Medicaid, SSI) to prevent gaps in care.
• Incorporate reentry goals and legal compliance into AOT Treatment Plans with the care team.
• Serve as liaison between care coordination team and community supervision officers to coordinate responses to violations.
• Conduct reentry risk/needs assessments and integrate findings into treatment planning.
• Connect participants to reentry employment resources and housing programs.
• Coordinate housing placements with housing specialists and reentry housing programs.
• Facilitate peer-based reentry support groups and provide psychoeducation on supervision and court processes.
• Document all activities in the EHR with HIPAA-compliant accuracy and timeliness.
• Participate in team meetings and mandatory trainings and contribute to quality assurance.
