Job summary

Seattle Children's is seeking an Anticoagulation Lead to advance clinical care across inpatient and outpatient settings, lead policy development, and supervise pharmacotherapy initiatives within the anticoagulation service.

Under pharmacy leadership, you will coordinate multidisciplinary efforts, monitor drug therapy and outcomes, educate staff and families, and drive quality improvement with emphasis on safe transitions of care and regulatory compliance.

Qualifications

• Graduation from an accredited School of Pharmacy.
• Licensed by the Washington Department of Health as a pharmacist.
• Completion of ASHP-accredited PGY1 residency or BPS Board Certification preferred.

Responsibilities

• Clinical Care & Coordination: Optimize anticoagulation therapy outcomes for patients.
• Policies, Procedures & Governance: Develop and maintain evidence-based policies and guidelines for anticoagulation therapy.
• Workflow Advancement: Translate clinical operations into safe workflow designs.
• Education & Staff Competency: Maintain anticoagulation competency program for staff pharmacists and educate patients and families.
• Data Tracking & Quality Improvement: Monitor adverse drug events and evaluate performance for improvement.
