Job summary

Seattle Children's is seeking an Anticoagulation Lead to advance clinical care across inpatient and outpatient settings, lead policy development, and supervise pharmacotherapy initiatives within the anticoagulation service.

Under pharmacy leadership, you will coordinate multidisciplinary efforts, monitor drug therapy and outcomes, educate staff and families, and drive quality improvement with emphasis on safe transitions of care and regulatory compliance.

Qualifications

• Graduation from an accredited School of Pharmacy.
• Licensed by the Washington Department of Health as a pharmacist.
• Completion of ASHP-accredited PGY1 residency or BPS Board Certification preferred.

Job description

Lead pharmacists serve as organizationally recognized owners of programmatic work which may be scoped entirely within pharmacy or may include collaboration with multidisciplinary leaders of the specific program. Under the supervision of applicable pharmacy leadership, each lead develops, implements and sustains ongoing programmatic support. Examples of programs requiring lead pharmacist support include but are not limited to: CDTA programs, training programs for pharmacy residents, complex medication production programs and services lines providing care for complex patient populations. The applicable supervisor or manager in pharmacy provides direction as to the specific elements of accountability within an individual program.

Pharmacists provide pharmaceutical care for a wide range of pediatric patients. Responsibilities include provision of cognitive services such as attending multidisciplinary rounds, managing therapy under prescriptive authority protocols, collaborating on guidelines of care, precepting residents and pharmacy students, pharmacokinetic monitoring, providing drug information and educating medical professionals, patients and families. Other duties include order verification, drug distribution and staff supervision.

Key Accountabilities For The Anticoagulation Lead Include

Clinical Care & Coordination

• Optimizes anticoagulation therapy outcomes for individual patients managed by Seattle Children’s Anticoagulation Service.
• Reviews drug levels and relevant laboratory parameters; makes appropriate recommendations for adjustment to drug therapy.
• Documents pharmaceutical care services and other clinical management activities.
• Coordinates anticoagulation therapy between inpatient and outpatient settings.

Policies, Procedures & Governance

• Participates in the development and maintenance of evidence-based policies, procedures, and clinical guidelines for anticoagulation therapy as co-chair of the Anticoagulation Safety subcommittee for P&T.
• Assures institutional adherence to regulatory requirements and evolving national patient safety goals for anticoagulant therapy.
• Supports structural alignment between approved CDTA scopes, clinical workflows, and EHR functionality to accurately reflect operational intent.

Workflow Advancement

• Collaborates with multidisciplinary leadership including Pharmacy, Medical Staff, Quality, and Information Services to translate clinical operations into safe workflow designs.
• Proactively identifies and mitigates system-level risks, gaps, or failure modes where workflow execution misaligns with clinical policy.
• Drives the implementation of specific pharmacy-driven electronic interventions, such as automated patient check-ins .

Education & Staff Competency

• Maintains anticoagulation competency program for staff pharmacists.
• Establishes and oversees processes to ensure that patients and their caregivers receive and comprehend culturally appropriate anticoagulation education, particularly at each transition of care.

Data Tracking & Quality Improvement

• Monitors and analyzes adverse drug events, including major bleeding, thrombosis, and preventable medication errors.
• Evaluates organizational performance against key clinical safety measures annually to systematically isolate and implement opportunities for improvement.

Required Education And Experience

Graduation from an accredited School of Pharmacy. Two (2) years of pharmacist experience in programmatic area.

Required Credentials

Licensed by the Washington Department of Health as a pharmacist.

Preferred

Completion of ASHP-accredited PGY1 residency. BPS Board Certification in the specific area of practice, if offered. Licensed by the Washington Department of Health as a pharmacist preceptor.

Compensation Range

$142,991.00 - $214,487.00 per year

Salary Information

This compensation range was calculated based on full-time employment (2080 hours worked per calendar year). Offers are determined by multiple factors including equity, skills, experience, and expertise, and may vary within the range provided.
