Qualifications

• Pharmacist licensure in Washington state required at hire.
• Board of Pharmacy Specialties certification within 2 years of hire.
• Board of Pharmacy Preceptor License required when eligible.
• PGY-1 Pharmacy Residency or equivalent education/experience.
• PharmD or B.Pharmacy degree required.

Responsibilities

• Provide collaborative practice services with the healthcare team for patient care.
• Utilize registries and reports to identify target populations and optimize therapy outcomes.
• Support population management and disease/medication education.
• Contribute to a culture of patient-focused, whole-person care at Providence Centralia Hospital.
