Job summary
Providence Centralia Hospital is seeking a Senior Clinical Pharmacist to collaborate with physicians and care teams to deliver coordinated, high-quality patient care. You will participate in patient care and disease state management to optimize therapy and outcomes.
You will use registries and performance data to identify target populations, educate patients, and support a culture of safety and continuous improvement within Providence.
Qualifications
- Bachelor's degree in Pharmacy or PharmD from an accredited School of Pharmacy.
- Postgraduate Year One (PGY-1) Pharmacy Residency or equivalent education/experience.
- Washington Pharmacist License upon hire.
- Certification from Board of Pharmacy Specialties within 2 years of hire.
- Board of Pharmacy Preceptor License upon hire where applicable.
- 3 years pharmacy practice experience with at least 2 years related experience in lieu of PGY-1.
Responsibilities
- Provide collaborative practice services in collaboration with the healthcare team.
- Utilize registries for chronic diseases and reports/tools to identify target populations and enhance therapeutic outcomes through education and drug therapy optimization.
