Qualifications

• Bachelor's Degree in Pharmacy from an accredited School of Pharmacy.
• PharmD in Pharmacy from an accredited School of Pharmacy.
• Postgraduate Year One (PGY-1) Pharmacy Residency or equivalent education/experience.
• Washington Pharmacist License upon hire.
• Board of Pharmacy Specialties certification within 2 years of hire.
• Board of Pharmacy Preceptor License - when eligible and where applicable.
• 3 years Pharmacy practice experience with at least 2 years of related experience in lieu of PGY-1.

Responsibilities

• Provide collaborative practice services in collaboration with the healthcare team.
• Utilize registries/tools to identify target populations and improve therapeutic outcomes.
