Job summary

California Department of State Hospitals in Sacramento is seeking a Referral Coordinator - Hybrid Telework to support the Patient Management Unit. Job Description And Duties

Under the supervision of the Supervisor I Staff Services Manager I in the Patient Management Unit (PMU), the Analyst I Staff Services Analyst (SSA) will process referrals by reviewing the required documentation and coordinating with county partners, DSH staff across facilities, contractors, and external stakeholders. Responsibilities are expected to show broader knowledge of managing an assigned caseload of patient referrals, and organizational and/or departmental requirements while the scope of work increases. The Analyst ISSA will regularly monitor their assigned caseload to ensure timely processing of referrals in relation to court mandated timeframes
