Job summary

Trinity Health in Muskegon, MI is seeking an Ambulatory Pharmacy Technician to support an Anticoagulation Clinic and other ambulatory services. The role involves triaging calls, scheduling, referrals, and maintaining patient records as part of a pharmacist-led care team.

Qualified candidates will have a high school diploma (or equivalent) and Michigan licensure (LPhT) with national certification (CPhT) or eligible within six months, plus prior pharmacy experience. Full-time, day shift.

Qualifications

• Minimum high school education or equivalent.
• Preferred ASHP/ACPE-accredited technician training.
• Licensed Michigan LPhT and nationally certified CPhT, or eligible within 6 months.

Responsibilities

• Triage patient phone calls and direct requests to care team.
• Coordinate patient scheduling, rescheduling, and follow-up appointments.
• Process and track patient referrals to ensure timely access to care.
• Maintain accurate patient documentation in the EMR.
• Support pharmacist-managed anticoagulation and ambulatory programs.
• Perform patient outreach and care coordination tasks.
• Monitor work queues to ensure timely completion of tasks.
