Job summary

Hackensack Meridian Health seeks a Pharmacy Navigator I to support Riverview Medical Center and Bayshore Medical Center. The role ensures timely delivery of prescriptions, assists with discharge planning, and counsels patients on medication access and insurance considerations.

The position requires a Pharmacy Technician license, 180-day certification window, and strong communication skills. Comprehensive benefits are offered, with competitive hourly pay.

Qualifications

• High School diploma or GED required.
• Excellent written and verbal communication skills essential.
• Proficient with Microsoft Office and Google Suite.

Responsibilities

• Establish greeting to patients and explain services available via HMH Pharmacies.
• Perform prescription order entry as needed.
• Obtain patient authorization to process prescriptions.
• Collaborate with floor nurses, discharge planners, case managers, social workers, physicians and staff.
• Identify and resolve insurance or financial barriers to medication access.
• Ensure confidentiality and privacy in all pharmacy activities.
• Coordinate pickup or delivery of prescriptions to patients or care areas.
• Support Ambulatory Pharmacy staff and perform related duties as needed.
