Job summary

Hackensack Meridian Health is seeking a Pharmacy Navigator I to support Riverview and Bayshore Medical Centers. You will coordinate prescription processing, ensure timely discharge medications, and collaborate with nurses, physicians and case managers to resolve barriers to access.

The role emphasizes patient counseling, privacy compliance, and safe medication delivery. The position requires a Pharmacy Technician license (or eligibility) with strong communication skills and proficiency in MS

Qualifications

• High School diploma/GED required.
• Excellent written and verbal communication.
• Proficient with Microsoft Office and Google Suite.

Responsibilities

• Greet patients and explain services available via HMH Pharmacies.
• Perform prescription order entry duties as necessary.
• Obtain patient authorization to process prescriptions.
• Collaborate with floor nurses, discharge planners, case managers and physicians to ensure timely medication delivery.
• Coordinate patient pickup or home delivery of prescriptions.
• Ensure offers for pharmacist counseling and provide pharmacist contact information.
