Hackensack Meridian Health seeks a Pharmacy Navigator I to support discharge and specialty pharmacy patients, ensuring rapid, safe access to medications as they leave the hospital or go home. This role combines pharmacy technician duties with care coordination across teams to resolve access issues.

The ideal candidate will have a high school diploma or GED, a Pharmacy Technician License, and be prepared to obtain a PTCB certification within 180 days of starting.

Qualifications

• High School diploma or GED required.
• Excellent written and verbal communication skills.
• Proficient computer skills (Microsoft Office/Google Suite).

Responsibilities

• Gathers necessary information and prioritizes tasks for discharge and specialty meds.
• Performs prescription order entry and obtains patient authorization as needed.
• Addresses insurance, financial, or clinical challenges to ensure timely medication access.
• Ensures OBRA-compliant counseling offers and patient education needs are met.
• Coordinates distribution of medications to patients at discharge or home.
• Adheres to privacy, safety, and regulatory policies and assists Ambulatory Pharmacy staff.
