Job summary

Hackensack Meridian Health is seeking a Pharmacy Navigator I to support discharge and specialty medication processes. You will gather patient information, process prescriptions, and coordinate with clinical teams to ensure timely medication access.

Qualifications

• High School diploma or GED required.
• Excellent written and verbal communication skills.
• Proficient computer skills including MS Office and Google Suite.

Responsibilities

• Greet patients and explain services available via HMH Pharmacies.
• Process prescription orders and obtain patient authorization.
• Collaborate with floor nurses, discharge planners, case managers, physicians and other teams to ensure timely discharge or clinic appointments.
• Identify insurance coverage and resolve medication access issues for patients.
• Adhere to privacy and compliance policies to protect patient confidentiality.
• Coordinate patient pickup or delivery of medications and provide pharmacist counseling information.
