Job summary

ChenMed is seeking an Ambulatory Pharmacist to optimize medication therapy within primary care clinics, operating under a collaborative practice agreement. The role focuses on reducing polypharmacy, improving adherence, and managing chronic diseases to enhance patient outcomes and quality metrics.

The Pharmacist collaborates with physicians, nurses, and care teams to deliver evidence-based, individualized care while ensuring safety, efficacy, and cost-effectiveness.

Qualifications

• PharmD degree required.
• Active pharmacist license in good standing.
• Pharmacy residency or ambulatory care certification preferred.
• Minimum two years of clinical experience in primary/ambulatory care.
• Immunization and medication therapy management certification a plus.
• BLS certification within first 90 days.

Responsibilities

• Medication management and optimization within protocol-driven ambulatory care.
• Conduct comprehensive medication reviews addressing safety and cost.
• Manage chronic diseases to improve control and outcomes.
• Coordinate care transitions and resolve medication discrepancies.
• Educate patients on medication use and adherence strategies.
• Order and interpret labs to monitor safety and effectiveness.
• Collaborate with physicians and care teams on protocols.
• Support department quality improvement initiatives.
