Job summary

ChenMed is seeking an Ambulatory Pharmacist to optimize medication therapy within our primary care clinics. You will work under a collaborative practice agreement, focus on reducing polypharmacy, enhance adherence, and improve outcomes across care teams.

You will lead medication reviews, support chronic disease management, and coordinate care transitions to prevent readmissions while adhering to evidence-based protocols. A PharmD and active license are required.

Qualifications

• PharmD required.
• Valid pharmacy license in good standing.
• Minimum two years of clinical experience in primary/ambulatory care.
• BLS certification within first 90 days of employment.

Responsibilities

• Medication management and optimization within protocol-driven framework.
• Comprehensive medication reviews for safety, efficacy, cost, and adherence.
• Chronic disease management (diabetes, hypertension, anticoagulation, asthma).
• Care coordination and transitions to prevent readmissions.
• Patient education on medication use, side effects, and adherence.
• Lab testing and monitoring to ensure safety and effectiveness.
• Collaborative practice with physicians to adjust medications as appropriate.
• Administrative support and contributions to quality improvement.
