Responsibilities

• Collaborate with physicians to optimize medication therapy.
• Conduct comprehensive medication reviews and address safety/efficacy.
• Manage chronic diseases and coordinate transitions of care.
• Order and interpret lab results to monitor therapy.
• Educate patients on medications, adherence, and safety.
• Support care protocols and independent medication adjustments when appropriate.
• Assist with quality improvement initiatives.
