Job summary

ChenMed LLC is seeking an Ambulatory Pharmacist to optimize medication therapy in primary care clinics. You will work under a collaborative practice agreement to reduce polypharmacy, improve adherence, and manage chronic diseases to enhance patient outcomes and quality metrics.

Responsibilities include collaborating with physicians and care teams, performing comprehensive med reviews, and educating patients on safe medication use.

Qualifications

• PharmD required with valid license in good standing.
• Registered Pharmacist (RPh) required; PharmD preferred.
• Ambulatory care or residency certification preferred.
• BLS certification within first 90 days is required.

Responsibilities

• Collaborate with providers to optimize medication therapy within protocol.
• Conduct comprehensive medication reviews for safety and efficacy.
• Manage chronic diseases to improve control and outcomes.
• Coordinate care transitions and reconcile medications on discharge.
• Educate patients on medication use, adherence, and safety.
• Order and interpret relevant lab tests to monitor therapy.
