Qualifications

• PharmD degree required.
• Valid state pharmacy license required.
• Ambulatory care residency or BCACP certification preferred.
• Minimum two years of relevant clinical ambulatory care experience.
• BLS certification within first 90 days of employment.
• Immunization and MTM certification is a plus.

Responsibilities

• Collaborate with providers to optimize therapies within protocol-driven framework.
• Conduct comprehensive medication reviews addressing safety, efficacy, cost considerations, side effects, and adherence.
• Proactively manage chronic diseases to improve control.
• Facilitate transitions of care and prevent readmissions.
• Educate patients on medication use, safety and adherence strategies.
• Order, interpret labs to monitor therapy and safety.
• Follow care protocols and support quality initiatives.
