Job summary

ChenMed in the United States is seeking an Ambulatory Pharmacist to optimize medication therapy in primary care clinics under a collaborative practice agreement. You will reduce polypharmacy, enhance adherence, and manage chronic diseases to improve CMS Star and HEDIS scores while working with physicians, nurses and care teams.

The role emphasizes evidence-based, individualized care, patient education, and cost-effective treatment.

Qualifications

PharmD degree required.
Valid state pharmacy license required.
Ambulatory care residency or BCACP certification preferred.
Minimum two years of relevant clinical ambulatory care experience.
BLS certification within first 90 days of employment.
Immunization and MTM certification is a plus.

Responsibilities

Collaborate with providers to optimize therapies within protocol-driven framework.
Conduct comprehensive medication reviews addressing safety, efficacy, cost considerations, side effects, and adherence.
Proactively manage chronic diseases to improve control.
Facilitate transitions of care and prevent readmissions.
Educate patients on medication use, safety and adherence strategies.
Order, interpret labs to monitor therapy and safety.
Follow care protocols and support quality initiatives.
