Job summary

HarmonyCares in the United States seeks a Pharmacist – Care Team to deliver clinical medication management for home-based patients, collaborating with our primary care team to optimize therapies and transitions of care.

You will review records, participate in high-risk discussions, develop monitoring plans, and provide education to patients and staff, while documenting in the EMR and contributing to quality improvement.

Qualifications

• PharmD required and licensed in the state(s) of practice.
• 3–5 years ambulatory care experience preferred.
• Strong communication and collaboration skills.
• Proficient with Visio, SharePoint, MS Teams, and Office Suite.
• Ability to work independently in a fast-paced environment.

Responsibilities

• Provide comprehensive medication management as part of the care team.
• Review records to resolve drug therapy problems.
• Lead comprehensive medication reviews in high-risk populations.
• Develop monitoring and treatment plans with the care team.
• Document in EMR and ensure data accuracy.
