Job summary

Option Care Health is seeking an entry-level Clinical Pharmacist to provide safe, compliant pharmacy services in the home care setting. You will collaborate with nursing staff, assess patient suitability for home care, and educate patients and caregivers on medication use and safety.

You will design and monitor pharmacotherapy plans, adjust therapy as needed, and ensure accurate dispensing and labeling in line with policies and USP standards.

Qualifications

• Pharmacist licensure and graduation from an accredited school of pharmacy.
• Knowledge of pharmacotherapy and regulatory standards.
• Strong communication and patient education skills.

Responsibilities
