Job summary

Option Care Health seeks a pharmacist to determine patient suitability for home care and to design, monitor, and modify pharmacotherapy plans for home care patients. You will educate patients and staff, ensure continuity of care, and dispense medications following policy.

Requirements include a PharmD, active state license, and experience in sterile IV admixture or MTM. Travel up to 5% within state may be required.

Qualifications

• Experience providing customer service to internal and external customers.
• Basic level skill in Microsoft Word for document handling.
• Basic level skill in Microsoft Excel for formatting and calculations.
• Basic skill level in Microsoft PowerPoint for slide preparation.

Responsibilities

• Determine suitability of patients for home care.
• Collect and organize patient information for home care suitability.
• Assess patients' suitability for home care per Option Care policies.
• Design and monitor pharmacy care plans for home care patients.
• Recommend modifications to prescribed therapy or monitoring parameters.
• Provide medication-use education to Option Care staff, home care patients and caregivers.
• Ensure continuity of pharmaceutical care between home and other patient-care settings.
• Prepare and dispense medications following Option Care policies and procedures.
