Job summary

Option Care Health is seeking a Clinical Pharmacist to join our home-based care team. You will provide safe, compliant pharmacy services in patients’ homes and coordinate with Nursing to educate patients, caregivers, and staff on medication use.

As an entry-level clinical pharmacist in an alternate setting, you will monitor progress toward goals, assist in MTM activities, and help ensure continuity of pharmaceutical care across home care and other settings.

Qualifications

• Experience providing customer service to internal and external customers, including meeting quality standards for services.
• Basic-level skills in Microsoft Word (docs, formatting) and basic Excel for data handling.
• Basic proficiency in Microsoft PowerPoint for preparation of presentations.

Responsibilities

• Determines suitability of patients for home care and collects necessary information.
• Designs, monitors and evaluates patient-specific pharmacy care plans for home care patients.
• Maintains and calibrates medication administration and related equipment used at home care sites.
• Provides drug information responses to health care providers and patients; pursues CEU opportunities to maintain competency.
• Ensures compliance with state/federal laws, DEA regulations, and internal QA processes.
• Participates in safety, confidentiality, and accuracy standards across home care operations.
