Job summary

HarmonyCares in Troy, Michigan seeks a Pharmacist – Care Team to provide clinical care to our home-based patient population. You will deliver comprehensive medication management in collaboration with the primary care team.

The role requires PharmD, ambulatory care experience, and licensure in the state. You will review records, develop monitoring plans, participate in medication reviews, educate patients and staff, and document in the EMR.

Qualifications

• Doctor of Pharmacy (PharmD) degree.
• Active pharmacist license in state of practice.
• PGY1/PGY2 or equivalent ambulatory care experience.
• Proficient with MS Office and EMR systems.
• Strong communication and teamwork skills.

Responsibilities
