ChenMed is hiring an Ambulatory Pharmacist to join our patient-centered team in Florida. You will optimize medication therapy within primary care clinics, under a collaborative practice agreement, focusing on reducing polypharmacy, improving adherence, and managing chronic diseases to elevate CMS Star and HEDIS metrics.

You will partner with physicians, nurses, and care teams to deliver evidence-based, individualized care, emphasize safety and cost-effectiveness, and participate in lab testing

Qualifications

• PharmD required and licensed to practice in Florida.
• Ambulatory care residency or BCACP preferred.
• Immunization and medication therapy management certification a plus.

Responsabilités

• Collaborate with physicians to optimize medication therapy within protocol-driven care.
• Conduct comprehensive medication reviews for safety, efficacy and adherence.
• Manage chronic diseases and monitor therapies to improve outcomes.
• Coordinate transitions of care and resolve med discrepancies during hospital discharges.
