Qualifications

• Pharmacist licensure in good standing and PharmD required
• Pharmacotherapy knowledge for primary care and ambulatory care settings
• Strong clinical decision-making and problem-solving skills
• Excellent communication and patient education abilities
• Ability to travel locally up to 50%
• English fluency in speaking and writing
• Independent judgment and collaborative practice experience

Responsibilities

• Collaborate with healthcare providers to optimize medication therapy within primary care clinics
• Conduct comprehensive medication reviews and address safety, efficacy, cost, and adherence
• Manage chronic diseases to improve control and outcomes
• Coordinate transitions of care and resolve medication discrepancies on discharge
• Provide patient education on medications, side effects, and adherence strategies
• Order and interpret labs to monitor safety and effectiveness
• Work with care teams to implement care protocols and independent adjustments per protocol
• Support quality improvement initiatives and physician consultations
