Job summary

ChenMed is seeking a Medication Adherence Pharmacist to embed in primary care clinics, enhancing adherence and chronic disease control under a collaborative practice agreement. The role partners with physicians, nurses and care teams to improve medication use and CMS Star/HEDIS measures.

The ideal candidate will have a PharmD, a current license, and experience with managed care; immunization certification is a plus. Travel up to 10% may be required in the market.

Qualifications

License in good standing with the state Board of Pharmacy.
PharmD degree; residency preferred but not required.
Experience in managed care settings is a plus.
Immunization certification is a plus.

Responsibilities

Outreach to patients at risk for non-adherence focusing on CMS quality measures.
Review prescriptions for safety and efficacy; adjust therapy per protocol.
Collaborate with care teams to optimize adherence and outcomes.
Perform comprehensive medication reviews addressing cost, access, and side effects.
Track adherence KPIs and report on measures like PDC and polypharmacy.
Provide daily verbal and written consultations on medication therapy.
