Job summary

ChenMed LLC is seeking a Medication Adherence Pharmacist to be embedded within primary care clinics. You will proactively manage adherence, chronic disease control, and quality performance metrics under a collaborative practice agreement, partnering with physicians, nurses, and care teams to optimize medication use and patient outcomes.

The role requires advanced clinical judgment, effective patient engagement, and the ability to implement evidence-based interventions to reduce hospitalizations

Qualifications

• Must have a Pharmacist license in good standing
• PharmD degree required; residency preferred but not required
• Collaborative Practice Experience or Ambulatory Care experience
• Experience supervising/support Pharmacy Technicians
• Managed care experience is a plus
• Immunization certification is a plus
• Medication Therapy Management or Ambulatory Care cert is a plus

Responsibilities

• Outreach to patients at risk for non-adherence focusing on CMS quality measures
• Review and verify prescriptions and medication orders for safety and efficacy
• Adjust medication therapy under a collaborative practice agreement
• Perform comprehensive medication reviews addressing cost, access and adherence
• Collaborate with providers to implement care plans and ensure timely refills
• Track and report adherence KPIs (PDC, polypharmacy, specialty adherence)
