Job summary

Requires PharmD, active license, and ambulatory care experience. Travel up to 50% may be needed; strong communication and decision making are essential.

Qualifications

• PharmD degree and current license in good standing.
• Ambulatory care/PCP clinical experience preferred.
• Certification in immunization or MTM is a plus.

Responsibilities

• Collaborate with providers to optimize medication therapy per protocol.
• Conduct comprehensive medication reviews for safety, efficacy and cost.
• Manage chronic diseases such as diabetes and hypertension.
• Coordinate transitions of care to prevent readmissions.
• Educate patients on medication use and adherence strategies.
• Order and interpret labs to monitor therapy effectiveness.
• Support quality initiatives and collaborative practice protocols.
