Qualifications

Responsibilities

• Perform telephone triage and care coordination (≈90%).
• Coordinate care with multiple specialists, hospitals, and community resources.
• Support clinic visits including patient/family education and care of medical devices.
• Partner with providers and interdisciplinary teams to support medically complex patients.
• Document and communicate nursing plans of care and patient status.
