Responsibilities

• Telephone triage and care coordination (approx. 90%).
• Coordinate care with multiple specialists, hospitals, and community resources.
• Support clinic visits including patient/family education and care of medical devices (e.g., trachs, feeding tubes, G-tubes).
• Specimen collection and other in-clinic support.
