Responsibilities

• Conduct comprehensive nursing assessments across age groups and conditions.
• Implement and monitor plans of care in alignment with policies and regulations.
• Provide patient and family health education on health promotion and disease management.
• Coordinate care and transitions within the hospital and with community services.
• Advocate for patients and families, guiding them through ambulatory services and resources.
• Document observations, care plans, and outcomes in the medical record.
