Job summary

UCSF Health in San Francisco is seeking an Ambulatory Pediatric Heart Center Nurse Care Coordinator to assess, plan, coordinate, implement and evaluate patient care in ambulatory settings. You will advocate for patients and liaise with insurers, inpatient/outpatient services, families, and healthcare teams to ensure timely coordination of care.

The role coordinates outpatient pediatric cardiology services, assists with insurance authorizations, ensures access to medications and home health

Qualifications

• Five (5) or more years of recent pediatric cardiac inpatient nursing experience.
• Knowledge of Pediatric Congenital Heart Disease.
• Current California RN licensure.

Responsibilities

• Coordinate outpatient services for a panel of pediatric cardiology patients.
• Assist with insurance authorizations and prior medication authorizations.
• Coordinate with home health, DME and medication refill requests.
• Provide patient education and family teaching.
• Manage clinic flow and assist with in-clinic procedures and care coordination.
