Job summary

UCSF Health in San Francisco is seeking an Ambulatory Pediatric Heart Center Nurse Care Coordinator to assess, plan, coordinate, implement and evaluate patient care in ambulatory settings. You will advocate for patients and liaise with insurers, inpatient/outpatient services, families, and healthcare teams to ensure timely coordination of care.

The role coordinates outpatient pediatric cardiology services, assists with insurance authorizations, ensures access to medications and home health

Qualifications

• Five (5) or more years of recent pediatric cardiac inpatient nursing experience.
• Knowledge of Pediatric Congenital Heart Disease.
• Current California RN licensure.

Responsibilities

• Coordinate outpatient services for a panel of pediatric cardiology patients.

Under general supervision, guided by the policies and procedures of the Medical Center and Ambulatory Services, and in collaboration with providers and administrative management, the Ambulatory Pediatric Heart Center Nurse works autonomously, utilizing the nursing process, to assess, plan, coordinate, implement and evaluate patient care within the ambulatory setting. S/he will utilize a high level of clinical knowledge to fulfill patient needs, ensure efficient patient care, and provide patient education when necessary. The Nurse Care Coordinator will act as a patient advocate and as a liaison between insurance companies, inpatient/outpatient services, patients/families and all other necessary healthcare services to manage and facilitate efficient and timely coordination of care. In addition, the Clinical Nurse Care Coordinator will assist in the development of patient care standards and practices.

The Nurse Care Coordinator will act as a patient advocate and as a liaison between insurance companies, inpatient/outpatient services, patients/families and all other necessary healthcare services to manage and facilitate efficient and timely coordination of care. In addition, the Clinical Nurse Care Coordinator will assist in the development of patient care standards and practices. The Pediatric Heart Center Nurse Care Coordinator is responsible for coordinating outpatient services and needs for a panel of pediatric cardiology patients and providers in the ambulatory setting. His/her responsibilities include both in clinic procedures, managing the Outpatient Heart Center clinic flow as well as assisting with insurance authorization and prior medication authorizations, contacting home health companies, assuring DME and other home supplies are adequate, assisting with medication refill requests, patient teaching and assisting in the overall coordination of timely patient care. This position is 100% in-person onsite. This position support patient care primarily in San Francisco and as needed in Oakland and satellite clinics.

Qualifications

Required Qualifications

• Five (5) or more years of recent pediatric cardiac inpatient nursing experience
• Ability to obtain CCS paneling within a year of hire
• Bachelor’s Degree in Nursing (BSN)
• Knowledge of Pediatric Congenital Heart Disease
• Strong heart rhythm interpretation experience
• Strong PIV insertion skills
• Experience performing Bubble studies
• This position requires flexibilityto orient and work at all UCSF Medical Center locations.

Preferred Qualifications

• Master’s Degree in Nursing (MSN)
• CCS paneled
• Recent experience working in an out-patient/ambulatory setting
• Experience with an Electronic Medical Record system, EPIC/APeX strongly preferred
• Microsoft Outlook Experience

Licensure/Certifications

• Current licensure as a Registered Nurse in the State of California.
• Current AHA ACLS Certification
• Current AHA BLS Certification
• Current AHA PALS Certification
