Advocate Health in Winston-Salem, NC, is seeking a dedicated ambulatory infusion pharmacist to support nine clinic locations within the Hematology & Oncology Ambulatory setting. This is a full-time, daytime role with Mon-Fri hours.

The ideal candidate will hold a PharmD or BS in Pharmacy, maintain an active North Carolina pharmacist license, and possess immunization and credentialing readiness. Strong communication and collaboration are essential to ensure safe, evidence-based patient care.

Qualifications

• Graduate of an accredited School of Pharmacy with a B.S. in Pharmacy or Pharm.D. is required.
• Current state pharmacist license is required in good standing through North Carolina Board of Pharmacy.
• Immunization and non-vaccine injectable certification required based on area of practice.
• Credentialing and privileging required based on area of practice.

Responsibilities

• Oversee pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens.
• Use patient education techniques to provide counseling to patients and caregivers on therapy, adherence, and safety.
• Design, monitor, and evaluate patient-specific pharmacotherapeutic regimens based on biomedical literature.
• Review and interpret medication orders and prescriptions accurately.
• Assist in formulary adherence and medication-use process management; collaborate with healthcare team.
